Waiver of Liability for Yoga, Mindfulness & Meditation

I acknowledge that I am participating in classes or services during which I will receive information and instruction about meditation, mindfulness, and yoga. I understand that yoga includes physical movements and I recognize that I may also choose to do physical movements, such as sitting, standing, and walking during a meditation or mindfulness session. As is the case with any physical activity, the risk of injury is always present and cannot be entirely eliminated. I am aware that participation in a sport or physical exercises, such as yoga, mindfulness, or meditation, may result in accident or injury, and hereby assume the risk connected with any participation for myself in any yoga, mindfulness or meditation class or services with Nicole Dooley.

Yoga, mindfulness, or meditation is not a substitute for medical attention, examination, diagnosis, or treatment. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness program, including yoga, mindfulness, or meditation.

I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities. If I do have any limitations or medical issues I will ensure I will adhere to my physician’s instructions and not do any poses that will cause any further injury.

I understand that personal results will vary and there are no expressed or implied guarantees or warranties of results.

I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against Nicole Dooley, any affiliates associated with Nicole Dooley, and any facility or location in which the yoga, mindfulness or meditation program is held.

I specifically agree that Nicole Dooley shall not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of, any kind of personal injury, property damage, or loss of any kind resulting from or related to my use of the facilities or participation in any exercise or activity within Nicole Dooley’s Program, and I agree to hold Nicole Dooley harmless from same.

I do hereby consent and agree that Nicole Dooley has the right to take photographs and/or videos of me during classes. These photos and videos may be used by Nicole Dooley to improve her classes. Any promotional use of video or photos depicting myself will only be used in any promotional matter only with a promotional usage release obtained in addition to this waiver.

Unless otherwise requested, I understand that my email address will be included on Nicole Dooley’s monthly newsletter list, and my email address will not be shared with anyone else without permission. If I choose to pay via debit or credit card I acknowledge that my contact details may be collected in a client directory.

I have read the above release and waiver of liability and fully understand its contents.

I understand that this waiver may be updated or modified at any time. I voluntarily agree to the terms and conditions stated above.

Child waiver of Liability for Yoga, Mindfulness & Meditation

I acknowledge that my child is participating in classes or services during which they will receive information and instruction about meditation, mindfulness, and yoga. I understand that yoga includes physical movements and I recognize that my child may also choose to do physical movements, such as sitting, standing, and walking during a meditation or mindfulness session. As is the case with any physical activity, the risk of injury is always present and cannot be entirely eliminated. As parent or guardian of the minor child participating in class, I am aware that participation in a sport or physical exercises, such as yoga, mindfulness, or meditation, may result in accident or injury, and hereby assume the risk connected with any participation for my child in any yoga, mindfulness or meditation class or services with Nicole Dooley.

While, as a parent, I am welcome to take pictures and videos depicting myself or my own child participating in class, it is never okay to capture or share other participants' faces or names.
